Before the session, a sheet to sign.

If you have booked a session with me, the release and the privacy notice are filled in and signed before the shoot. Choose the right form: private individual, minor or company.

You can fill it in here, print and sign it, or download the blank PDF and write it by hand.

How it works

  1. Fill in the fields: the document updates as you type.
  2. Press "Print" (or "Download as PDF" to save it) and print all pages, single-sided.
  3. Tick the consent boxes by hand, write place and date and sign where required, including the signature approving the specific clauses.
  4. Scan or photograph all pages and upload them in the "Already signed?" form below, or send them to [email protected], or hand them over before the session.

The website does not save the details you enter: they stay on your device only. It is a tool I offer free of charge to my clients to fill in and print the release. Consent boxes, place, date and signatures are filled in by hand on the printed sheet.
